+ Is the SARS-CoV-2 virus saliva Test covered by insurance?

Reimbursement is at the discretion of a patient's insurance provider.

+ How do I submit this Test to my insurance provider for reimbursement?

After you register, you will receive a Test Requisition Form. When you get your results, you will receive a PDF Lab Report.

You can submit your Test Requisition Form, your Test receipt, and your Lab Report with your results to your insurance provider to request reimbursement. It is recommended you submit all documents together. Look below for "Where do I find my Lab Report, Test Requisition Form, and Test receipt to submit to insurance?"

Your Test receipt (numbered 1 below) will include:

• Type of product or service
• Amount paid for Test
• Description of expenses

Your Test Requisition Form (numbered 2 below) will include:

• Date of service
• Customer name and birthdate
• Name and NPI of the service provider
• Name and information about the lab
• Procedure Code (CPT Code) and explanation
• Diagnostic code (ICD-10 Code) and description
• Type of product or service

Your Results PDF (numbered 3 below) will include your results.

How to Submit a Claim for Reimbursement:

1. Download a claim form from your health insurer’s website or call the number on your insurance card to ask for a claim form for services provided by an out-of-network provider. (see form links for some insurance companies below).
2. Return that claim form along with your receipt, your Test Requisition Form, and your Lab Report. Some companies may want it scanned and emailed; others may want it faxed. Others may offer online submission. Please follow their exact instructions. We have listed some links to forms from popular insurance companies below.

Unfortunately, we cannot help you appeal if you do not receive reimbursement, and we cannot guarantee that your insurance company will reimburse you for all or part of the cost of the test.

United Healthcare: How to submit a claim | Submit a claim online by logging into your account through this link and clicking on the COVID-19 form. When you do so, attach your receipt, Test Requisition Form (sent to you when you register), and your Lab Report (sent with your results). OR, print out this COVID-19 form, fill it out before or after printing it, and mail it along with the claim details (in your Test Requisition Form) and your receipt to the address on the back of your health plan ID card.

+ Where do I find my Lab Report, Test Requisition Form, and receipt to submit to insurance?

Within our secure portal, you can access your PDF Lab Report and your Test Requisition Form by

1. (On a Computer) Clicking the documents tab on the left or (On a phone) clicking the documents icon on the bottom.
2. Navigating to your conversation with Biotech Accelerated and clicking on the attachments sent in the messages.
3. Clicking on the doctors tab, clicking on the carrot next to "Biotech Accelerated," and lookin for the links in your conversation with Biotech Accelerated. *Test Requisition Form is titled "screening-submission" and Lab Report is titled "covid--test-results"
